Muktad Prayers for the Departed

Prayers will begin at 6:00 PM SHARP. Doors will open at 5:00 PM, and a potluck dinner will follow the prayers (ZAMWI will provide soft drinks and paper products).
Ervad Brigadier Behram Panthaki, Ervad Kurush Dastur, and Mobedyar Zen Panday will conduct the prayers. Each family may submit up to 5 names for inclusion in the prayers. We prefer that you submit your names and donation online (suggested donation is $6 per name), but you can also download and mail this form by July 28th. If mailing the form, your donation checks should be made payable to “ZAMWI”. Phone messages will not be accepted.
Flowers may be offered at the ceremony, and you may bring a framed picture of your departed individual(s) for the Muktad table.
Lastly, please let us know in the form below what you will be bringing for the potluck. Even if you are not submitting any names for the prayers, please fill this section out so that we can plan the potluck dinner.
